MEDICAL INFORMATION

School Health Exam Requirements for State Compliance:

Kindergarten:
  • Physical Exam with immunizations
  • Dental Exam
  • Vision Exam
2nd Grade:
  • Dental Exam
6th Grade:
  • Physical Exam with immunizations
  • Dental Exam
New students who did not attend an Illinois school last year:
  • Physical Exam with immunizations on an ILLINOIS school form
  • Vision Exam

Dates of exams that meet the requirements:
Physical dated after 9/1/2009 is valid to meet the requirement for the 2010-11 school year.                 

Dental exam dated after November 15, 2009 is valid to meet the requirement for the 2010-11 school year.

Vision exam dated after 9/1/09 is valid to meet the requirement for the 2010-11 school year.

Dental Requirements:

State of Illinois requirements are as follows:

All students entering kindergarten, second and sixth grades are required to have dental exams by May 15 of each of these years.
