MEDICAL INFORMATION

School Physical Requirements for State Compliance:

  • All students entering kindergarten and 5th grade need a new physical prior to start of school year.
  • All Students transferring from out of state will need a new physical on the Illinois form. Medical form can be      found on web site.
  • All physicals are good for one year prior to start of school.



    • Dental Requirements:

    State of Illinois requirements are as follows:

    All students entering kindergarten, second and sixth grades are required to have dental exams by May 15 of each of these years.
