Extra-Curriculuar Activities and Athletic Programs
Sports Physical Form
In order for students to participate in a sport, an updated physical is needed. All sections, including exam by a physician, must be filled out. If a student does not have a valid physical on file, they will not be able to try out for the team. The physical is valid for 13 months.
